Classroom Library Collection

Please bring these documents with you to Open House. You may have already submitted them at ChildFind screenings. We can make copies if needed.

1.    Copy of Birth Certificate

2.    Copy of Social Security Card- if available

3.    Immunization Record (from Health Department or Physician)

4.    Proof of Physical (can be checked & signed on Immunization Form)

5. Any medical diagnosis reports that may be important to school nurse or developmental needs.

All other registration paperwork will be completed at Open House.

1. Backpack (large enough to fit a folder)

2. Pick one: 24 count Crayola Crayons OR 10 pack of Crayola Markers

3. 1 plastic pocket folder without brads and 1 plastic folder with brads

4. Box of Tissues

5. Pick one: small size Dixie brand cups, baby wipes, OR roll of paper towels

6. Snacks- We stock the pantry with snacks to share & then we notify parents when our snack cabinet is low.  Ideas: crackers, chips, cookies, animal crackers, gummy snacks, etc…..

7. Change of Clothes-seasonally appropriate

8. Pullups, diapers & wipes, if needed
